Position Description: Provide individualized, group and consultation services related to all aspects of client health and specialized medical needs, utilizing a team-based, multi-disciplinary approach.

Qualifications:

Physical Requirements: · Frequent movement within the program including but not limited to: kneeling, bending, sitting, lifting, and positioning of students. · Operation of adaptive and specialized equipment. · Ability to use computer. · Ability to see, speak and hear · Ability to transfer consumers from and to wheelchair · Ability to complete and implement physical training requirements, determined by KDC · Ability to tolerate sustained periods of walking, standing, sitting

Responsibilities:
§ Coordinate medical services with each members PCP § Obtain reports and approval of treatment plans from the PCP
§ Provide direct care and training in relevant areas § Responsible for all aspects of medication administration. § Administer first aid as needed. § Develop and/or maintain up-to-date medical records. § Work cooperatively/collaboratively with medical and other community providers to ensure coordination of health supports and services. § Communicate with families / residential programs as needed regarding client health needs. § Participate in the continuing interdisciplinary evaluation and review of each member’s needs and status § Document in each members record any recommendations resulting from periodic reviews § Complete and maintain documentation as required by KDC and all applicable funding or regulatory agencies.
§ Report, monitor and document all staff work-related injuries.
