Position:

Nursing Coordinator

Status:

Part time/Full time, Exempt (min. 22.5 hrs/wk)

Position Description: Provide individualized, group and consultation services related to all aspects of client health and specialized medical needs, utilizing a team-based, multi-disciplinary approach.

 

Qualifications:

  • MA Licensure as Registered Nurse.
  • Experience in a program serving persons with developmental disabilities
  • Demonstrated ability to effectively participate in multi-disciplinary team based approach.
  • Excellent organizational, interpersonal and communication skills.
  • Supervisory experience – min. 2 yrs.
  • Able to meet required competency level in Microsoft Office Software.
  • 1 yr. of experience in community health setting

Physical Requirements:

·         Frequent movement within the program including but not limited to: kneeling, bending, sitting, lifting, and positioning of students.

·         Operation of adaptive and specialized equipment.

·         Ability to use computer.

·         Ability to see, speak and hear

·         Ability to transfer consumers from and to wheelchair

·         Ability to complete and implement physical training requirements, determined by KDC

·         Ability to tolerate sustained periods of walking, standing, sitting

 

 

Responsibilities:

  • Oversee all health-care services provided to the members at the program

§         Coordinate medical services with each members PCP

§         Obtain reports and approval of treatment plans from the PCP

  • Advise the program director and the day habilitation staff of any medical problems that may hinder a member’s progress or that may be aggravated by activities at the program
  • Ensure availability of appropriate specialized equipment for health and medical needs.

§         Provide direct care and training in relevant areas

§         Responsible for all aspects of medication administration.

§         Administer first aid as needed.

§         Develop and/or maintain up-to-date medical records.

§         Work cooperatively/collaboratively with medical and other community providers to ensure coordination of health supports and services.

§         Communicate with families / residential programs as needed regarding client health needs.

§         Participate in the continuing interdisciplinary evaluation and review of each member’s needs and status

§         Document in each members record any recommendations resulting from periodic reviews

§         Complete and maintain documentation as required by KDC and all applicable funding or regulatory agencies.

  • Report, complete and maintain all documentation relating to client accidents.

§         Report, monitor and document all staff work-related injuries.

  • Participate in development of necessary medical, health and safety policies.
  • Participate in hiring and provide orientation, training and ongoing supervision and support of nursing staff.
  • Participate in professional development activities and training necessary to enhance skills and maintain certification.
  • Assist in outreach and program fundraising activities.
  • Demonstrated strengths in interpersonal and communication skills
  • Ability to analyze, evaluate and prioritize
  • Ability to advise, influence, exercise good judgment, and make thoughtful decisions
  • Demonstrated ability to train effectively, adapting style to meet needs of those requiring training    
  • Coordinate the provision of health care supports through training, monitoring required medical services and coordinating recommended follow-up
  • Provide direct medical support for individuals as required
  • Establish and monitor medication administration systems
  • Coordinate medical support for all new referrals
  • Oversee maintenance of all required documentation
  • Provide medical/health care assessment for DHSP as part of interdisciplinary evaluations
  • Provide monthly progress notes for all individuals
  • Ensure compliance with regulatory practices and health-related procedures
  • Obtain reports and approval of treatment plans from primary physicians
  • Perform other job-related responsibilities as assigned
