Position Description: As a member of the Early Intervention and Early
Intervention Partnerships Maternal Child Health Team, the Social Worker,
provides assessments, service coordination, and referral for issues related to
the developmental ans psychosocial needs of each family and child.
Qualifications:
- Master’s Degree and at least an LCSW (licensed
clinical social worker) licensed by the Massachusetts Board of Registry
for Social Work, or equivalent degree in psychology, or current licensure
as a Licensed Mental Health Counselor (LMHC).
- Knowledge of child growth and development.
- Knowledge of family systems theory, methods of
anticipatory guidance, and counseling techniques.
- Home visiting and supervisory experience
- Able to meet required competency level in
Microsoft Office Software.
- Must have a valid Massachusetts Driver’s License,
registration and safe driving record.
- Bilingual/bicultural preferred.
Physical Requirements:
- Ability to meet with families and/or individuals
within their homes.
- Ability to sit for extended time periods.
- Frequent phone use required.
- Ability to use a computer.
Responsibilities:
- Participate as a member of the Early Intervention
and EI Partnership Team in program planning and delivery of services.
- Provides assistance and consultation to program
staff
- Participates in ongoing evaluations of children
and families when needed and contributes to the development of the Family
Care Plan and Individualized Family Service Plan.
- Consults with team members on case management and
referral services for families, including psychosocial child and family
assessments, parent training and support.
- Facilitates staff, team meetings, group and
individual supervision.
- Completes and maintains documentation as required
by KDC and all applicable funding or regulatory agencies.
- Demonstrates ability to form trusting,
non-judgmental and supportive relationships with new parents, their
children and other family members.
- Demonstrates respect for diverse family
structures, practices, and beliefs, particularly related to health and
parenting.
- Performs other job-related responsibilities as
assigned.
- Assists in outreach and program fund-raising
activities.
