Position Description: As a member of a transdisciplinary team and in
partnership with families, promote optimal developmental skills through the
provision of activities to meet outcomes outlines on Individualized Family
Service Plans. (IFSP)
Qualifications:
- Must have required degree
and requirements as defined in Early Intervention Operational Standards. (See
Below)
- Excellent organizational,
interpersonal and communication skills.
- Able to meet required
competency level in Microsoft Office Software.
- Must have a valid
Massachusetts Driver’s License, registration and safe driving record.
- Bilingual/bicultural
preferred.
Physical Requirements:
- Ability to meet with
families and/or individuals within their homes.
- Moderate travel required
- Ability to sit for extended
time periods.
- Frequent phone use
required.
- Ability to use a computer.
Responsibilities:
- Participates as a member of
the transdisciplinary team in program planning and delivery of services.
- Participates in
intake/screening of new clients.
- Participates in initial
assessments and annual evaluations of children and families contributing
to the development of IFSP.
- Maintains safety and
medical standards according to agency policy and procedures.
- Maintains expected level of
productivity.
- Provides case management
and referral services to assigned caseload, including participation in
interagency case conferences.
- Provides home visits to
assigned caseload, for purposes of providing developmental stimulation,
parent training and support.
- Responsible for implementation
of IFSPs.
- Participates in
parent/child and/or child groups.
- Provides co-treatment with
other members home and center-based.
- Provides parent education
activities.
- Provides teaching and
consultation to team members.
- Completes narrative reports
of assessments, progress reports, IFSPs, and discharge summaries.
- Maintains appropriate
communication with physicians, clinics, and community agencies
participating in each child’s care.
- Coordinates
transition/termination process for clients, including referrals to Chapter
766 services when appropriate.
- Maintains assigned client
records according to program procedures.
- Demonstrates the ability to
work collaboratively in a team approach to service provision.
- Maintains compliance with
all Kennedy-Donovan Center policies and procedures, DPH regulations and
requirements.
- Participates in
professional development activities and training necessary to maintain
relevant certification/licensure in accordance with their professional
discipline and in accordance with EI Operational Standards.
- Participates in in-service
training, team meetings, group and individual supervision.
- Assists in outreach and
program fundraising
- Performs other job-related
responsibilities as assigned.
Degree and Requirements:
- Occupational Therapy: Current licensure as an Occupational Therapist
by the Massachusetts Board of Registration in Allied Health Professions.
- Physical Therapy: Current licensure as a Physical Therapist by the
Massachusetts Board of Registration in Allied Health Professions.
- Speech and Language
Pathology:
a)
Current licensure by the
Massachusetts Board of Registration in Speech-Language Pathology and Audio logy
and a Certificate of Clinical Competence (CCC) granted by the American Speech,
Language and Hearing Association or
b)
Currently in clinical fellowship
year prior to being granted a CCC.
