Position Description: As a member of the School’s management team, the Family Support Coordinator is responsible for the coordination of supportive services to families, facilitating outreach and referrals to community health and human services and oversight of behavioral plan development and implementation.

 

Qualifications:

·         Master’s degree in Social Work, Psychology, Counseling or related field.

·         Excellent organizational, interpersonal and communication skills.

·         Experience in family counseling and behavior management.

·         Able to meet required competency level in Microsoft Office Software.

 

Physical Requirements:

·         Frequent movement within classroom including but not limited to: kneeling, bending, sitting, lifting, and positioning of students.

·         Ability to meet with families and/or individuals within their homes.

·         Ability to sit for extended time periods.

·         Frequent phone use required.

·         Ability to use computer.

 

Responsibilities:

·         Schedule home visits for family meetings at a minimum two times per year.

·         Schedule at least quarterly phone contact to families for assessment of need for supports.

·         Coordinate and facilitate parent advisory groups at least four times per year.

·         Assist families in identifying their needs and resources and preferences for support.

·         Coordinate and assist families to obtain needed supports through other community services.

·         Work cooperatively/collaboratively with other community service providers and LEA special education departments to ensure coordination of supports and services.

·         Provide resource and referral information to families.

·         Assess individual student needs related to social, emotional and behavioral development.

·         Develop appropriate behavior plans that outline strategies to promote positive behavior development.

·         Provide support to classroom staff in management of students’ challenging behaviors.

·         Participate as member of multi-disciplinary team in providing ongoing support and consultation regarding individual students.

·         Participate in staff, team meetings, group and individual supervision.

·         Participate in development of Individual Education Plans, including family assessments.

·         Complete and maintain documentation as required by KDC and all applicable funding or regulatory agencies.

·         Participate in development of student progress reports as scheduled.

·         Assist in outreach and program fundraising activities.

·         Perform other job-related responsibilities as assigned.
