JOB DESCRIPTION
Position: Home Support Coordinator
Reports To: Program Coordinator
Status: Fulltime

Position
Description: Facilitate and coordinate the provision of home-based supports and
community resources for individuals with developmental disabilities, utilizing
a team-based approach. Promote individual growth in the areas of choice and
decision-making, community membership, relationships and personal accomplishments.
Qualifications:
·
Bachelor's degree in Social Work, Psychology or Human
Service related field.
·
LSW, LSWA or eligibility.
·
Strong interpersonal and communication skills.
·
Commitment to community based supports to individuals
with developmental disabilities.
·
Commitment to family-based services.
·
Able to meet required competency level in Microsoft
Office Software.
Physical
Requirements:
·
Ability to meet with families and/or individuals within
their homes.
·
Extensive travel required.
·
Ability to sit for extended time periods.
·
Frequent phone use required.
·
Ability to use computer.
Responsibilities:
·
Facilitate and coordinate home placement and support
services for individuals based
upon individual needs and preferences.
·
Supervise assigned home care and support providers.
·
Assist providers in identification of training and
support needs.
·
Participate in and/or facilitate training and support
forums for home care and support providers.
·
Promote individual choice and self-expression based upon
quality service principles.
·
Provide direction and support to home care and support
providers in facilitating individual social inclusion opportunities.
·
Promote activities and opportunities leading to personal
growth and accomplishment.
·
Participate in the planning process to ensure adequate
support in area of health and. safety.
·
Support and monitor consumer funds and financial
transaction.
·
Assist in development of Individual Support Plan.
·
Facilitate and coordinate access for assigned individuals
to needed supports, including generic community services.
·
Work cooperatively/collaboratively with other community
service providers and medical professionals to ensure coordination of supports
and services.
·
Complete required documentation of service delivery.
·
Assist in outreach activities including development of
materials and program presentations.
·
Participate in staff meetings, group and individual
supervision and case reviews.
·
Provide emergency beeper coverage services during
non-office hours and respond to emergencies as needed.
