A vacancy is announced in the following full time position:
Individual Support Case Manager Milford
Position Description: Facilitate and provide home and community supports for
individuals with developmental disabilities, utilizing a team-based approach. Promote
individual growth in the areas of choice and decision-making, community membership,
household management skills, relationships and personal accomplishments.
Qualifications:
- Bachelors degree preferred.
- Excellent organizational, interpersonal and communication skills.
- Strong interpersonal and communication skills.
- Experience with and commitment to community based supports to individuals with
developmental disabilities.
- Valid drivers license.
- Able to meet required competency level in Microsoft Office Software.
Physical Requirements:
- Extensive travel required.
- Ability to meet with individuals within their homes.
- Ability to sit for moderate time periods.
- Frequent phone use required.
- Ability to use computer.
Responsibilities:
- Provide direct skill training, case management, supportive counseling and crisis
intervention as directed by the individual.
- Promote individual choice and self-expression based upon quality service principles.
- Promote activities and opportunities leading to personal growth and accomplishment
- Participate in the planning process to ensure adequate support in the areas of health
and safety.
- Support and monitor individual funds as appropriate.
- Assist in the development of the Individual Support Plan.
- Facilitate and coordinate, with the individual, access to needed supports, including
generic community services.
- Provide support and assistance to individuals necessary for them to develop and maintain
relationships with others.
- Provide transportation as appropriate in response to individual schedules and desired
activities and/or appointments.
- Work cooperatively/collaboratively with other community service providers and medical
professionals to ensure coordination of supports and services.
- Ensure that individuals are treated with respect and dignity at all times, and maintain
confidentiality regarding their records and other information
- Ensure completion and maintenance of documentation as required by KDC and all applicable
funding agencies.
- Participate in staff meetings, group and individual supervision, and case reviews.
- Provide emergency beeper coverage services as assigned during non-office hours and
respond to emergencies as needed.
- Perform other job-related responsibilities as assigned.
