EI Specialist-Educator- SLP-OT-PT- Southbridge EI Program
The salary range is: Per salary scale
Position Description: As a member of a transdisciplinary team and in partnership
with families, promote optimal developmental skills through the provision of activities to
meet outcomes outlined on Individualized Family Service Plans. (IFSP)
Qualifications:
- Must have required degree and requirements as defined in Early Intervention Operational
Standards. (See attached.)new ceis credentials.htm
- Excellent organizational, interpersonal and communication skills.
- Able to meet required competency level in Microsoft Office Software.
Physical Requirements:
- Ability to meet with families and/or individuals within their homes.
- Moderate travel required.
- Frequent phone use required.
- Ability to use computer.
Responsibilities:
- Participates as a member of the transdisciplinary team in program planning and delivery
of services.
- Participates in intake/screening of new clients.
- Participates in initial assessments and annual evaluations of children and families
contributing to the development of IFSP.
- Maintain safety and medical standards according to agency policy and procedures.
- Maintain expected level of productivity.
- Provides case management and referral services to assigned caseload, including
participation in interagency case conferences.
- Provides home visits to assigned caseload, for purposes of providing developmental
stimulation, parent training and support.
- Responsible for implementation of IFSPs
- Participate in parent/child and/or child groups.
- Provide co-treatments with other team members home and center-based.
- Provide parent education activities.
- Provide teaching and consultation to team members.
- Completes narrative reports of assessments, progress reports, IFSPs, and discharge
summaries.
- Maintains appropriate communication with physicians, clinics, and community agencies
participating in each childs care.
- Coordinates transition/termination process for clients, including referrals to Chapter
766 services when appropriate.
- Maintain assigned client records according to program procedures.
- Demonstrates the ability to work collaboratively in a team approach to service
provision.
- Maintain compliance with all Kennedy-Donovan Center policies and procedures, OCCS/DPH
regulations and requirements.
- Participate in professional development activities and training necessary to maintain
relevant certification/licensure in accordance with their professional discipline and in
accordance with EI Operational Standards.
- Participate in inservice training, team meetings, group and individual supervision.
- Assist in outreach and program fundraising activities.
- Perform other job-related responsibilities as assigned.
